What to Know About Your Next Appointment

1. All patients and parents/guardians entering the building MUST arrive wearing a fabric face covering. We regret that we are unable to provide face masks to patients and their guardians, due to a national shortage. If you do not already have a face covering, the Surgeon General has created a quick tutorial on how to easily make a no-sew mask with an old t-shirt and rubber bands. 2. The following form must be completed ON THE DAY OF YOUR APPOINTMENT, PRIOR TO ENTERING THE BUILDING. Please note that it needs to be completed for ANY INDIVIDUAL ENTERING THE BUILDING, including parents and guardians.

3. Once you have arrived and are parked, please check in by sending a text with the patient’s name to 888-845-3074

4. We will text you when it’s time for you to enter the building.

5. In an attempt to minimize the number of people in the building at one time, it is ideal for patients to enter on their own. Children can be accompanied by a parent or guardian.

*If you are accompanying a child and you will have other children with you, please let us know as soon as possible. We understand that it’s not always possible to find care for other children. Letting us know in advance will allow us to make arrangements for safe social distancing.

6. A member of our team will greet you at the door, take your temperature, and ask a few quick questions before bringing you in for your appointment.

7. If your child has come in on their own for the appointment, their Orthodontic Assistant will reach out to you to let you know what was done at the appointment and to schedule your child’s next appointment. If your child has a phone, the Assistant and your child will call you with Facetime. If your child does not have a phone, the Assistant will send a text to you.

8. Please arrive 15 minutes before your scheduled appointment time. This will help us to see all patients as close to their appointment time as possible.

9. Please brush your teeth before arriving for your appointment.
